* I authorize the doctor(s) and staff of Parkdale Veterinary Wellness Center to provide services (medical, surgical, lodging) for my pet(s).
* I understand emergency veterinary advice is available 24/7 by calling the regular office number, 231-723-8998. A veterinary professional will help determine if your pet is having an emergency and refer you to the nearest pet ER if needed.
* I agree to pay the bill in full at the time services are rendered.
* I agree Parkdale Veterinary Wellness Center may take pictures and/or videos of my pet for continuing education, medical publications, promotion, social media, etc. These images will have no identifying information about me or my family associated with them. They may contain my pet's name. I will claim no ownership of or authority over said images.
* I understand that veterinary medicine is not an exact science and no guarantee can ever be given for an outcome concerning my pet's health.
* If my account goes into collections, I agree to reimburse Parkdale Veterinary Wellness Center the fees of any collection agency, which may be based on a percentage at a maximum of 24% of the debt, and all costs and expenses, including reasonable attorney’s fees, we incur in such collection efforts.
*If your check is returned or an ACH payment fails for any reason a $25.00 fee will be applied to your account in accordance with state law
